If you are a veteran Brebeuf coach, we ask that you complete the following: Background Check Form Emergency Contact Information Form  Athletic Program Employee Information Form on an annual basis.  See links below.

New, or returning after a number of years absence? We need the following items from you.

Employee Information Form 

Employee Eligibility Form (I-9)
Must be submitted with proper ID's within three days of employment

Emergency Contact Information Form

Federal Tax Form  

WH-4 State of Indiana Tax Form

VIRTUS (Required Training for all staff) Information
Please follow the directions provided, register for a VIRTUS training,
and submit proof of attendance to H.R. (Rm. 196A). Thank you.
