Dear Parents of the Class of 2025 and Transfer Students,
I hope this message finds you safe and healthy. We are very excited to welcome our new students to
the Brebeuf Jesuit community, and for student-athletes, they will begin working with our coaches this summer.
While summer is important for sport-specific development, it is as important that students spend time with their families and friends, go on vacation, participate in service and immersion trips, and attend camps. All of our coaches value those experiences because it plays a key role in the education of the whole person.
Currently, we are planning for summer Wellness Center hours, team workouts, and open facility opportunities to begin on June 1, 2021. Below I have included details and logistics about the next steps that you will need to take in order for your student-athlete to participate in summer Athletics.
The transition from middle school to high school athletics can be challenging, but our coaches are here to ensure that it is a smooth and transformational experience.

Wellness Center Orientation
Wellness Center Orientation for all incoming Brebeuf Jesuit freshmen and transfer students will be on June 1-4, 2021, from 7:00-9:00 a.m. Student-athletes should plan on attending two of the four sessions in order to complete their orientation. Student athletes who have conflicts (e.g. vacation, camp) should contact Coach Clem at email@example.com to set up an alternate time to complete the orientation. The purpose of the orientation is to teach student-athletes how to safely use the equipment in the Wellness Center so that they are prepared for team strength and conditioning workouts. Student-athletes who have not completed the Wellness Center Orientation will not be allowed to participate in the strength and conditioning workouts with their team(s) until they have done so.
Student-Athlete Required Participation Forms and Registrations
Step One:Complete this Pre-Participation Physical History Form.
Step Two: Schedule a Physical Examination.
Take this Physical Examination Form with you to your appointment. Brebeuf Jesuit typically holds a “Physical Night” on campus that allows students and families to complete the necessary paperwork and registrations, but due to COVID-19 restrictions, that event will not occur this year. All students and families are instructed to see a primary care physician, DO, NP, or PA to complete this step.
The Pre-Participation Physical History Form and the Physical Examination Formneeds to be on file in the Athletic Department before the first organized team workout or Wellness Center orientation, whichever happens first.

Step Three: Set up a Family ID Account, and Complete the Registration
Go to: https://www.familyid.com/programs/2021-22-athletics-brebeuf-jesuit-prep. Student-athletes only have to register once each year, so there is no need to fill one out for each season. Please disregard any part of the registration process that shows an amount due or asks for payment.
Students cannot tryout until we have the Pre-Participation Physical History Form and the Physical Evaluation Form on file with the Athletic Department and you have completed the registration via Family ID. We realize that you most likely can’t get your physical examination completed right now, but you can complete the Pre-Participation Physical History Form and complete the Family ID registration now. Please don’t turn in your participation documents until you have completed all of the steps.
